The following information is required for your student’s registration.

We cannot offer services in our program until we have all of your documents.

Proof of Residency

At least one original document from each of the following categories must be given:

Category One- Parent/ Guardian ID
●      California Driver’s License
●      California Photo ID
●      Passport
●      Photo ID card issued by a government agency

Category Two- dated with the last 60 days
●      PG&E bill
●      Home telephone bill
●      Water bill
●      Cable television bill

Category Three
●      W-2 form
●      Property tax payment receipt
●      Copy of lease and most recent rent payment receipt

Medical Record

●      Copy of the insurance card of the child (Medi-Cal, etc.)
●      A copy of the child’s physical. (The check-up must have been administered in the past 12 months)
●      If you need to make an appointment with the doctor, please let us know the date of the doctor’s appointment.
